What Is Dialectical Behavior Therapy?

Developed in the 1980s by Dr. Marsha Linehan, dialectical behavior therapy is a cognitive-behavioral treatment initially developed to treat borderline personality disorder (BPD). BPD is a mental illness characterized by impulsive and self-destructive behavior, difficulty regulating emotions and mood, and issues with relationships. DBT has since been expanded to treat other mental health disorders, including but not limited to:

Depression

Bipolar disorder

Eating disorders

Post-traumatic stress disorder (PTSD)

The goal of DBT is to help patients learn how to regulate their emotions and improve their interpersonal skills to manage triggers, stressors, and relapses better. This allows patients to experience a more balanced and stable life. The “dialectical” in DBT refers to the duality of accepting both the patient’s current reality and their goals when setting treatment plans. The “behavior” refers to the focus on behavioral change rather than purely cognitive intervention.

How Does DBT Work?

During DBT treatment, patients learn skills in four main areas:

Mindfulness

Distress tolerance

Interpersonal effectiveness

Emotional regulation

Mindfulness is the practice of being present in the moment and accepting one’s thoughts and feelings without judgment. It helps patients become more aware of their thoughts, emotions, and behaviors as they arise, allowing them to better regulate and respond to these experiences.

Distress tolerance refers to the ability to tolerate difficult emotions and situations without engaging in self-destructive or impulsive behaviors. Patients learn how to cope with difficult emotions in a healthy way and how to avoid making things worse in the moment.

Interpersonal effectiveness refers to the ability to effectively communicate with others and negotiate in a way that meets one’s needs without causing harm to oneself or others. Patients learn how to assert themselves, set boundaries, and say “no” in a respectful and effective manner.

Emotional regulation is the ability to manage emotions in a healthy and productive way. It involves understanding the roots of one’s emotions and learning how to respond to them so that they don’t make things worse.

DBT treatment is typically provided in individual and group therapy sessions.

Treatment usually spans a six to eight-week time period; however, treatment may be extended based on clinical need.
